Please use the links below to complete the necessary forms to apply to have your educational activity certified for CME credit. This process helps to ensure the activity is planned, developed, implemented, and evaluated to ensure the CME is presented to learners in a way that is evidence-based, scientifically balanced, and free from commercial bias.

The ACCME's Standards for Integrity and Independence in Accredited Continuing Education have been adopted by the AOA and many other CME/CE accrediting bodies, and these guide the creation and evaluation of all CME activities accredited by LMU-DCOM.

All activities applying for CME credit should submit the following:

1) PDF icon LMU-DCOM CME Application for Clinical Partners - To complete, please save this fillable form on your computer; complete in full; and sign electronically. Alternatively, this form can be printed, completed and signed by hand, and scanned and sent via email.

2)  PDF icon LMU-DCOM CME Speaker Agreement and Disclosures - ALL speakers and moderators for an activity (including tumor boards and other weekly activities) must complete this document in full. Please follow the same instructions for completion as for #1.

3) PDF icon Disclosure Form for Planners - ALL planning committee members must complete and sign this form. Please follow the same instructions for completion as for #1.

4) CVs - all speakers, moderators and planners (anyone signing one of the above forms) must submit a CV (or resume).

5) Documenting the Practice Gap and Need - Planners should submit supporting documentation for the practice gap and need identified on page 2 of the CME Application. Documentation can be in the form of new guidelines; scholarly research on the topic; internal needs assessments/patient outcomes data; learner surveys from previous events, etc.
